Infant Reflux: Understanding GER vs. GERD
If your baby spits up frequently, you are not alone.
According to the Canadian Paediatric Society (CPS), regurgitation occurs in about 20% of infants at 1 month and peaks at 41% between 3 and 4 months. Most of these babies are “happy spitters” who will outgrow this naturally by their first birthday.
Is it GER or GERD?
It is important to distinguish between simple reflux and the disease:
- Gastroesophageal Reflux (GER): Normal spitting up or burping without pain. Most healthy infants do not require medication for GER.
- Gastroesophageal Reflux Disease (GERD): Occurs when reflux causes troublesome symptoms like poor weight gain, respiratory issues (coughing), or extreme irritability during feeds.
First Steps: Non-Pharmacological Management
The CPS recommends trying lifestyle modifications before moving to medication, as the risks of acid-suppressants (like infections or bone health concerns) often outweigh the benefits for healthy infants.
- Thickened Feeds: A 2-week trial of thickening breastmilk or formula can significantly decrease the frequency of vomiting.
- Cow’s Milk Protein Trial: Some “reflux” is actually a milk allergy. A 2-to-4-week trial of eliminating cow’s milk (from the mother’s diet if breastfeeding) or using a hydrolyzed formula may resolve symptoms
- Safe Sleep: Despite common myths, the CPS strongly advises against elevating the head of the crib or side sleeping, as these techniques increase the risk of SIDS. Infants should always sleep flat on their backs.
